Maniapoto Health Provider Alliance Submission Form

The Waikato DHB have requested feedback on an options paper from the Waitomo Community on a local model for primary care dated 13/07/10. They privided four options for consideration. The Maniapoto Health Provider Alliance (MHPA) have come up with a variation of their option 3 and have called it Option 5. Here attached is a form created by the MHPA for feedback primarily for those who support Option 5 as displayed in attachment. Down load fill out and return using one of two methods;
